Description:
About the Role
We are recruiting an experienced and confident Complex Care Case Manager to take overall responsibility for the day-to-day coordination and leadership of an established home-based care package in Dublin.
This is a hands-on leadership role. You will not simply coordinate the package from an office — you will need to understand complex care environments, have experience working directly with service users, and be confident leading a team of care staff supporting the client.
You will be responsible for ensuring the care package is safe, consistent, well-organised and operating to a high standard. You will work closely with the client, family, care team and wider professionals, providing clear leadership and responding appropriately when challenges arise.
The successful candidate must also be comfortable with out-of-hours availability when required, particularly where there is a crisis, staffing issue or urgent situation affecting the care package.
Key Responsibilities
• Lead and manage the care team supporting the package
• Take ownership of the overall day-to-day running of the care package
• Work directly with the service user and understand their individual needs, routines and support requirements
• Act as a key point of contact for the family, client and relevant professionals
• Provide clear direction, leadership and support to care staff
• Ensure staff follow care plans, risk assessments, routines and agreed procedures
• Monitor the quality and consistency of support being provided
• Address staff concerns, performance issues and challenges promptly and professionally
• Support staff with difficult situations and provide appropriate guidance
• Coordinate staff schedules, appointments and daily routines
• Assist with onboarding and integrating new staff into the package
• Maintain strong communication across the care team
• Respond appropriately to incidents, concerns and emerging risks
• Be available to provide support during out-of-hours situations or crises when required
• Escalate serious concerns to Clinical24 management and relevant professionals
• Coordinate appointments and engagement activities
• Manage household administration, shopping and organisation
• Manage petty cash and maintain accurate records
• Monitor household supplies and inventory
• Maintain a structured, safe and well-organised home environment
• Complete any additional tasks required to ensure continuity and stability of the package
About the Client
• Teenage female service user
• Based in Dublin
• Established home-based complex care package
• Support focused on structure, routine, consistency and engagement
Essential Experience & Skills
We are looking for someone with strong practical care experience combined with leadership ability.
You must have:
• Previous experience working directly with service users in a care or support environment
• Experience supporting individuals with complex needs
• Previous experience in complex care, disability, mental health, social care, residential care, community care or a similar setting
Experience supervising, coordinating or leading a team of care/support staff
• A strong understanding of person-centred care and maintaining professional boundaries
• Confidence managing challenging situations and responding calmly during a crisis
• Experience working with care plans, risk assessments and safeguarding procedures
• Strong communication skills with service users, families, staff and professionals
• Excellent organisational and administrative skills
• The confidence to hold staff accountable and address issues when standards are not being met
• Ability to work independently and take ownership of the package
• Strong decision-making and problem-solving skills
• Good computer skills and attention to detail
• Full driving licence and access to a car required
• Based in or near Dublin preferred
• RMN/Mental Health Nursing, Social Care, Disability or similar professional background would be advantageous
Out-of-Hours Availability
• This role requires someone who understands that complex care does not always operate within standard office hours.
• You must be willing to provide reasonable out-of-hours support when required, particularly during:
• Staffing emergencies
• Service-user crises
• Significant incidents or concerns
• Urgent family or professional queries
• Last-minute staffing or operational issues
You will have the support of Clinical24 management and will not be expected to manage serious situations alone. However, the successful candidate must be someone who is reliable, responsive and comfortable taking ownership when urgent situations arise.
Why Join Us?
• Leadership position within an established complex care package
• Opportunity to lead and develop a dedicated care team
• Consistent hours and ongoing work
